1. Can I book an appointment on-line?

Yes- please go to www.flourishhealthcare.ca and click on the Book Now button. This is for follow-up appointments only. For all new clients booking an initial appointment, please call 613-539-4576.

2. Why is there a fee?

Services by Nurse Practitioners working in independent practice are not covered by the Ontario Health Insurance Plan.

3. What does my fee include?

The fee includes the professional time that the Nurse Practitioner spends with you, and any relevant testing such as bloodwork. The fee also includes behind the scenes time that is spent on ordering and interpreting lab results, research into your unique case, and business overhead expenses- such as an electronic medical record system, fax machine, rent, staff, etc.

4. Why do I need to go for bloodwork or other diagnostic tests?

There is a saying: If you want to clean a house, you need to see the dirt. This is how we find clues as to where the problem lies, and oftentimes further diagnostic testing is necessary. This could be a stool test, or a complete hormone test for example. Using data points to track is helpful in seeing the progress in your case.

5. How do I get my blood requisition?

We will provide you with a lab requisition at your initial appointment. Further blood testing will be determined at your follow-up appointments and may need to be more or less often depending on your case. We will also upload the lab requisition into the outsmart portal in the documents section. You may log into app.outsmartemr.com to print this off.

6. How can I see my treatment plan?

You can log into app.outsmartemr.com and put in your email address and your password. This will take you into the outsmart portal where you can see your treatment plan and any other documents shared with you.

Note: if you have forgotten your password simply click on “forgot my password” to reset it.

7. Why are more frequent follow-up appointments in the beginning necessary?

We want to teach you the steps to be healthy on your own in order for you to achieve lasting results. It takes time to learn new ways and we will guide you.

There is a saying: the breakthrough is in the follow through- so this is essential in experiencing results.

8. How can I share documents and results with Flourish Healthcare?

You may upload your test results (ie. other blood tests, mammograms, abdominal ultrasound reports) to your outsmart portal. Just title it with the date and what test it is. Please let us know you if have done this- email anna@flourishhealthcare.ca.

9. How can I reach the Nurse Practitioner urgently?

We are not an urgent care clinic and do not provide emergency or acute care  appointments. Please go to your family physician, walk-in clinic, or go to the nearest emergency department. We are not able to respond to urgent requests or last minute prescription refill requests. It is up to you plan ahead and book an appointment for prescription refills at least one month before you run out completely.

10. Can I get my prescriptions refilled without an appointment?

No, please make an appointment for a proper assessment to ensure that it is safe for you to continue on the medication.

11. Can I email Flourish Healthcare with medical questions to receive medical advice?

No- Email is not a secure platform for sending private health related messages. In order for us to serve you optimally, please book an appointment to address your health questions.

12. What outcomes/ results can I expect and how do I get there?

You will experience an increased capacity to flourish in health and to be  resilient when things get challenging. Flourishing in health leads to flourishing in life.

You will get there by following through with your action plan, taking it one small step at a time. This will lead to the break through to achieving your
desired results.

13. How can I get the most out of my appointments with the nurse practitioner?

Come prepared with questions. Bring a notebook to write notes in. Be engaged in the process. Reflect on what small changes you can make to
advance your health forward.
